Connections is a cozy, intimate neighborhood for residents with the early to middle stages of Alzheimer’s and dementia and is designed for residents who are showing increasing signs of memory loss. We assess residents’ maximum cognitive ability and recall. Then, we “meet them where they are” and make the most of their strengths.

Residents in the early to middle stages of memory loss are able to independently participate in activities that give purpose to each day. Our secured Connections neighborhood features personalized programs that create routine and structure to help these residents feel safe and successful. By assuming most of the responsibilities of daily living, we create more time for residents to focus on whatever gives them comfort and pleasure. We heartily encourage families to visit regularly and to dine with their family members, as if they were sharing time together in their own home.

Our goal in the Connections neighborhood is to help residents be independently engaged in activities that give meaning and purpose to their days. We find or create ways to help residents feel comfortable, secure, respected and loved. Our entire team


weave professional care with heartfelt sensitivity into the everyday lives of residents. We help residents patiently with even the smallest of tasks; watch carefully for cues when we should offer more support; and retreat when appropriate to encourage as much independence as possible.


Care Available to Residents of the Connections Neighborhood:
  • Licensed nurses on-site 24 hours a day, 7 days a week
  • Individualized service plans
  • Primary care manager for each resident
  • Round-the-clock concierge/lifestyle assistance
  • Medication management, including injections, by a licensed nurse
  • Pharmacology review at move-in and quarterly thereafter
  • Physician office on-site
  • Customized approach to meals with adaptive equipment and dining assistance if needed
  • Specialized diets and hydration program
  • Rehabilitation services
  • State-of-the-art sensors to alert staff to needs or changes in routines that call for attention
  • Incontinence management
  • Coordination with hospice as needed
